Terms and Conditions:

1	Companion awards are nontransferable and must be used in conjunction with the member's paid ticket. Companion award travel seats are limited; check availability prior to booking paid ticket. Companion award travel must be booked directly with Midwest Airlines via phone. Limit one free companion award ticket per member per trip.
2	Seats are subject to award capacity controls. Availability is limited to maximum seating capacity. No overbooking will be allowed.
3	You will receive a Certificate by mail within 2-3 weeks for redemption when you travel. Pet travel is subject to capacity controls. A maximum of three pets may travel in the passenger cabin. The number of pets accepted for travel in the lower Pet Compartment is subject to aircraft size, weight limits and the number of pets already booked.
