Request Mileage Credit - Midwest Airlines

Request Mileage Credit Form

Mileage credit may take up to 45 days
to post to your account. To request credit
for past travel on Midwest Airlines, use
this form. You may also file requests for
your companions who did not receive mileage
credit for the same travel reservation.
Simply fill in the passenger name and
Midwest Miles account number. Their accounts
will be credited upon verification.

To receive mileage credit for flights
flown on a qualifying partner airline, send
a copy of your ticket receipt and boarding
passes to the Midwest Miles Service Center.
Questions regarding mileage credit requests
for non-airline participants such as hotels,
car rentals, and special services should be
submitted in writing within one year and must
include the Midwest Miles account number and
a copy of the member's ticket or other
appropriate receipts.
